STARDATE 90949.8: Explosion on Medical Decks during baryon sweep

Stardate: 90949.8Location:DS-13 Medical Deck CReport Submitted: Commander Rylen Yores

On or about Stardate 90949.8, during a routine baryon sweep being conducted by Starfleet OSHC, an explosion occurred on Medical Deck C, in Counselor Sedai's office. The baryon sweep was immediately halted. Due to systems being in low-power mode for the sweep, suppression systems did not operate properly, and as a result, the explosion caused a collapse in the deck. To control the fires, force fields were put up and the deck was vented. Due to the damage being localized, the decision was made not to evacuate personnel from the station, and the affected area had already been evacuated for the sweep.

After it was confirmed there was no longer a danger to personnel, a team was dispatched to investigate the affected areas. Upon arrival, the teams found severe structural damage to Medical Deck C, rendering Counselor Sedai's office unusable. All computer consoles on the deck were offline, due to breaks in power conduits. Initial inspection yielded a field diverter which appeared to be defective. The diverter was taken from the scene by Lt. Kermit, to be given a full forensic diagnostic in an attempt to determine the cause of the failure; it was turned over to an Engineering team. The baryon sweep will be postponed until the cause of the diverter malfunction can be determined, and a full diagnostic run on all other diverters to be used in the sweep to ensure there are no other defective units.

Damage control teams were dispatched to Medical Deck C to determine the full extent of damage, and to begin repairs. At this time, the duration of repairs is unknown.

Damage Analysis Report and Summary

INCIDENT PRECIS

Assigned baryon sweep team 21-RHO was completing scheduled staggered sweep of DS13's Medical Block. At roughly 50% completion as the sweep progressed from deck B to deck C, sensors picked up energy surges and console overloads on deck B. Baryon sweep was aborted at 62% and integrated systems were brought back online promptly to deal with the resultant explosions and fires.

No other locations on station reported damage or any anomalies.

SPECULATION OF CAUSES

Debris teams located several field diverter generators with defects on both decks B and C. As diverters registered as functioning according to pre-sweep diagnostics, assumption at this point is that the most likely point of failure is in the diverter deflector cells. As these are usually tested post-manufacturing, it is uncertain whether the batch of diverters are a result of a bad manufacturing accident or a work of sabotage. Final determination of such remains outside scope for this analysis.

The OSHC sweep team has been informed and required to perform extraneous tests to ensure all field diverters are working properly for future sweeps.

RESTORATION OF OPERATIONS

The most extensive damage is isolated between medical decks B and C, consisting primarily of administrative and office spaces. 26% of electronic equipment will need to be replaced. Estimated three to five days for clean-up and roughly two, two and a half weeks for full repairs.
