Commander t'Xereth of the Romulan Republic contingent came into the infirmary requesting evaluation for a headache. She expressed concern that the headache had been caused by unsolicited psionic contact from a Reman, a fellow member of the Republic contingent.

Neural scan confirmed the presence of elevated levels of neurochemical X-848 (see registry for Vulcan nomenclature). X-848 is expressed at extremely high levels in Vulcans and Vulcanoids suffering from Pa'nar Syndrome. Levels recorded in Commander t'Xereth are only slightly elevated, magnitudes lower than presents with Pa'nar Syndrome, but it does suggest unregulated psionic contact took place.

Following treatment, provided Commander t'Xereth with a copy of this report to use as necessary in filing a formal complaint.

Additional notes: The patients were rushed to sickbay after surprisingly falling unconscious after drinking green tea at the Starlight Cantina. I believe they had ingested the methanol through the tea and an investigation has been launched.

They had not ingested enough methanol to kill them,but it caused them difficulty with their breathing.The methanol had caused minor damage to their kidney,liver and lungs which had became physically apparent. The damage was minor and easily treatable.The patients are to be kept in sickbay for observation until the methanol is completely washed out their systems by being excreted from their Kidney's.

Additional notes:
Captain Konieczko suffered a disruptor burn during an incursion with a Klingon boarding party aboard the Vanguard. This office would like to note that the Captain refused treatment until negotiations with the allied Klingons were complete, which may be responsible for the complications to his injury.

This physician performed surgical repair of all affected organs, but the existing pancreatic structure was unsalvageable. Dr. Linna Drei successfully installed a prosthetic to perform the necessary functions until the cloned organ is available for transplant.

Though not strictly necessary, it is the recommendation of this physician that Captain Konieczko be confined to the medical bay until the organ transplant is complete. It should also be noted that the patient is permitted NO alcohol for two weeks; his painkilling medication level will be determined by Dr. Drei or myself.

Lieutenant Commander Zaldo struck a bulkhead during the Klingon incursion and suffered a burn on the same shoulder.

This physician reducted the shoulder and administered analgesics and anti-inflammatories, and applied burn ointment and a sealing agent. LCDR Zaldo has indicated she would like to have the scar removed. An appointment has been logged into the system approximating the completion of the healing process.

Additional notes: LCDR Callahan was brought into sickbay by Lieutenants T'kel and Shorran. Presented with superficial dermal burns, evidently from a laboratory accident. Pain was relieved by hypospray, and a course of dermal regeneration remedied tissue damage. LCDR Callahan, upon insisting on resuming her duties, has been advised to avoid activities that may further damage her hands, and will return in twenty four hours to ensure that there is no neurological damage.

Additional notes:

Ensign Coolbaugh was injured by an explosion while on duty in the secondary computer core and was unconscious upon arrival in sickbay.

The patient suffered third degree burns on both hands; fingertips on the right show eschar too significant for direct regeneration on first, third and fourth metacarpals. Approved surgical removal of affected digits and authorized replication of synthetic prostheses for future installation pending nervous system recovery. Regeneration units and burn gel packs were applied after necrotic tissue was removed.

The patient absorbed intense electrical current, damaging upper peripheral nervous system, disrupting oculomotor function and causing irregular heartbeat and respiration. The patient received recommended dose of ivoremin and remained on system support post-surgery until pulse and pressure stabilized. He remains on respiration assistance and under observation.

Several pieces of housing were removed from his face and upper torso. The wounds were treated with appropriate topical medication and closed by protoplaser to avoid regenerative inundation disorder.

Upon waking, Ensign Coolbaugh exhibited confusion, slurred speech and poor short term recall. These are symptoms of temporary neuropathic trauma and should pass within the next 24-36 hours.

It is the opinion of this physician that Ensign Coolbaugh is not fit to leave sickbay. Will advise regarding his return to duty when more data is available.

DIAGNOSIS: Body beginning to reject implants, leading to severe nanite loss, increased neural activity and total organ failure.

NOTES:

On Stardate 94272.5, Captain Fives came to visit me complaining of the fact that she had had to replace her implants on a frequent basis, something that was highly unusual. Upon closer examination, her body showed various levels of implant degradation, all worse than the previous examination and with some implants being dangerously close to complete failure. Her neural signs were high and her heart was pulsing at 201% above normal levels. Her blood tests confirmed that her nanite levels were down by 30% and dropping, and her nanites were all heavily damaged.

Upon informing the patient of the diagnosis, she, understandably, began to experience signs of fear which caused her to go into shock. I administered 30 cc's of Cardiostimulants and took a measure of the patient. Her neural signs were erratic, almost as if she was receiving mixed signals from elsewhere. To prevent further damage, I placed the patient into an induced coma and transferred her to the Intensive Care Unit for further treatment.



RECOMMENDATION: The patient seems to be rejecting her implants. With the patient's permission, the likely solution is to remove the implants. This will have to be confirmed by the patient beforehand. In the short term, we have to stabilise the patient enough to take her out of the coma.

Additional notes: Patient dropped off by Captain Foraker of the U.S.S. Nannietta Bacco, unconscious and with hand wound. Hand wound caused by sharp implement, possibly a knife, reportedly self-inflicted. Regenerated hand wound, began IV nutrients to unconscious patient for supplements to treat malnourishment.

Patient awoke six days later, anxious and uncomfortable at the restraint of the biobed. Administered improvoline to calm the patient when it was clear that there was no capability to reason with him, as there was no common language. Began crude attempts to communicate, patient cooperated. Was permitted to leave the biobed and he began tapping at the console to produce soothing musical tones. Have been unable to explain neurological scarring from detailed scans or lack of common language when, from all previous history, patient has been able to communicate in Federation Standard before.

Additional notes: Patient denied treatment for malnutrition and became verbally combative while staff attempted to ascertain the cause and reasoning of her injury and nutritional status. While she first seemed uncertain as to how her injuries came about calling them caused by a 'trip'. Patient was of the appropriate mental faculties to make the decision to be discharged.

Additional notes: Commander sh'Sonora dislocated her shoulder during a running of some form of Ushaan-tor practice holodeck simulation. At her own testimony, she states the injury occurred with the holodeck safeties engaged and that the incident was a result of both her and her holographic opponent pulling on the tethered pair of weapons simultaneously. Patient also expressed interest in further martial arts practice, a form under the title of Jeet Kune Do - a terran martial art. Due to an interest in said forms, patient stated symptoms of headaches likely a result from previous, multiple concussions. Further examination is advised, however patient showed no further signs of trauma to warrant concern.

Allergy panel shows positive result for pet dander. Patient advised to limit exposure to certain common species of mamallian pets, prescribed cordrazine as needed for symptoms. Patient informed “Borg Feline Hyper-Influenza” fictitious.
