SUMMARY

Patient is suffering from extremely high levels of neurological activity in all four major lobes. The occipital and parietal lobe show the highest signs of these neurological activity. This manifests itself through the patient losing focus when exhibited with strong emotional triggers.

Additionally the pituitary and pineal glands are secreting Zaraphoid and Fernayahide, which is uncommon to the Tesrin race.

After a careful analysis of the Tesrin race, it appears that the patient falls into the very small minority who exhibit signs of telepathic and empathic abilities. These abilities tend to develop during the pre-pubescent stages, delaying puberty in the patient. This explains the high neurological activities and unknown hormones in the body.

Other than the neurological activities listed above, the patient is of sound health. Continue to monitor telepathic development, neurological activity, and child development, especially since puberty is delayed because of the development of the patient’s telepathic ability.

The patient’s dual Andorian and Aenar heritage cause some minor issues that are worth noting. This manifests itself in the thinner blood, which is thin, even for Andorian standards. This, coupled with the lower heart rate, means that the patient should take care when doing overly strenuous activities.

The thinner blood also manifests itself to injuries; any cut means that the patient is susceptible to losing a lot of blood quickly if not treated. Hence, the patient always carries around a dermal regenerator in case of injury.

SUMMARY

Patient is healthy and fit for duty, with some reservations regarding an injury which happened less than a year ago. The Captain's last exam was a followup for surgery to replace ribs with artificial substitutes after they were damaged due to explosives eight months ago. The replacements have healed well, but bouts of phantom pain have occasionally occurred, and the left side of the body has had difficulty with feeling sensations of touch. Besides this, the patient has experienced little feeling in their left side as a result. The patient reports that they were sedated or in artificially induced coma for most of this time. Patient has been prescribed steroids and regular neurological therapy to help regain feeling in the left side and reduce phantom pain.

The patient further reports occasionally drowsiness as a result of large work loads. Recently this drowsiness has lead to mild feelings of being light headed. Captain Hill, however, feels that this feeling does not impact them in any significant way.

Tests were taken manually, without the use of the biobed medical diagnostic arch due to the patient’s claustrophobia but this has no effect on the outcome of the evaluation; a second opinion was given by Doctor J. The patient still shows signs of the effects of past trauma, but there are no physical or mental effects that hinder her condition. The hormonal suppression implant that was implanted on Stardate 93015.6 on vertebra C5 was in excellent condition and the patient has not experienced any ill effects from it.

Commander Leveson-Scott is in fine health, with her evaluation proceeding without incident. The patient showed minor signs of residual infection from when she was MIA on the planet in both her airways and bloodstream. The patient had been prescribed a course of Tazamadin when she had her last evaluation after rescue. She was administered another double dose during the evaluation which should clear up any residual signs. These do not have any impact on her ability to work.

Other effects from her MIA have all passed, such as any minor injuries, or any effects due to the atmosphere. The patient is in good neurological condition; although experiencing mild stress and trauma, this has not had an effect on her work.

The patient also was administered a pheromone blocker by representatives of the OSN via a hypospray in the two months between the last evaluation and the present one. This was a short short duration suppressant pre-approved by the Intelligence Department, and closer examination showed no lasting effects.

SUMMARY
Captain Nimitz has suffered a significant number of injuries during his career. Please see attachment for full list.
Of these injuries, four items remain concerns for ongoing medical treatment.

  1. His left eye, a prosthetic, requires annual testing and periodic recalibration. Failure to comply with this necessity could result in damage to his right eye through overcompensation. Currently functioning WNL.
  2. His artificial heart requires similar observation, but contains the SorelCor adaptability matrix; regardless of weight gain/loss, injury or illness, the heart should adjust automatically to meet the needs of the host.
  3. His dermal scarring is extensive, and while it does not yet inconvenience him, over the course of time it may become more cumbersome. He has declined treatment at this time.
  4. Perhaps of greatest concern, the mineral shards in his upper arms have become fused with the tissue and likely cause minor to moderate discomfort on vigorous use of the affected musculature. It is the opinion of this physician that the affected musculature could be replaced with natural or synthetic grafts. The CMO of the Hyperion has recommended against a procedure to remove them, citing potential nerve damage. It is the opinion of this physician that such nerve damage could be avoided by replacing the entire structure rather than attempting to remove the fragments alone. Perhaps this CMO is taking into account the subject’s apparent unwillingness to be out of service for the time necessary to recover from that procedure.
Despite the conditions listed, Captain Nimitz is rated capable of active duty.

Dr. sh’Zarath has been suffering periodic headaches of moderate intensity for the last few months. She has been successfully self-treating.

An in-depth scan revealed stress injury to the specialized sensory apparatus responsible for the Aenar telepathic ability. High populations of non-telepathic entities aboard the Axiom and DS13 have resulted in an involuntary overcompensation of effort to create functional telepathic links. If she were among her own kind, or positioned on a ship with a smaller crew, this would never have come up.

An inhibitor would also inhibit her ability to see. It is the opinion of this physician that daily temporary relief, during sleep or interspersed throughout the day, would eliminate the source of these headaches. Investigation and results will be forwarded to Dr. sh’Zarath aboard the Axiom.

SUMMARY
Ensign Steele was recently injured when an overloaded phaser exploded in his vicinity, causing concussive and lacerative damage localized primarily to his head. Subject has recovered ahead of previously scheduled estimate. This resulted in a mild chemical imbalance caused by prescription colcitrine, dispensed to subject to assist in recovery. Discontinuing the medication should resolve dizziness. Advised subject to return to infirmary if symptoms do not resolve or worsen.

SUMMARY
During a period of imprisonment by the Iconians, Commander Thiessen was subjected to a number of experiments by his captors. This included the introduction of multiple infectious agents into his system, presumably for purposes of measuring their effects on a human subject.
The medical team that treated him upon rescue is to be commended for their fine work; however, once the initial crisis had passed, Commander Thiessen received no follow-up care or examinations. As a result, the patient has developed a chronic hyper immune response that will require a series of organ transplants and a course of drug therapy to correct. Preparations for this procedure are now in progress, to be scheduled when the Commander returns from his next patrol.
Commander Thiessen is rated provisionally capable of fulfilling his duty on condition that the procedure takes place on schedule. It is the opinion of this physician that the CMO of the USS Baldur should be reprimanded for failing to assess the Commander’s health prior to this evaluation. The Commander has volunteered that he intentionally avoided visiting his CMO, but that is no excuse for neglect. An earlier diagnosis could have spared the patient additional organ damage, and a later one could have resulted in serious and potentially fatal complications.

SUMMARY

Captain Thiessen previously underwent a multi-organ transplant following [redacted] and shows no symptoms of rejection at this time. Unfortunately, he has developed high blood pressure bordering on hypertension levels. Considering his excellent physical conditioning, it is the opinion of this physician that external factors are more likely to be the trigger for this hypertension.

Discussed different lifestyle change options for reducing blood pressure. Capt Thiessen will also reach out to counseling to counter the effects of personal and administrative stress. Per regulations, the Captain’s blood pressure will be monitored weekly until it returns to safe levels.

SUMMARY
On the surface Ms. Tierno appears to be a perfectly healthy human. In fact one could almost say too perfect. Her respiratory and cardiovascular tests were above average, though not unfounded, it is uncommon. Scans from blood and saliva tests indicate previously detected mutation in the RNF216-IT1 gene is still absent. Confirming the previous medical evaluation.
Further detailed examinations of the subjects DNA and genetic samples indicated an oddity in cell age, possibly indicating artificial creation and additionally absence of blood type would support this theory. This is worth further investigation.

SUMMARY

Cadet Tierno is in excellent physical condition with her response and reaction times beyond those of what we might expect from a normal person.

The cadet’s previous medical reports have consistently mentioned a small genetic mutation on the RNF216-IT1 gene, on Chromosome 7. Whilst this mutation is minor and caused no problem to the patient, examination of the RNF216-IT1 gene now reveals no such mutation. A second opinion may be needed to confirm these findings, but this has no impact on the health of the cadet.

In regards to the patient’s amnesia, it has had little impact on the cadet’s duty and continues to not hinder her development. Her counselling sessions have been downgraded to annual check-ups.

SUMMARY
Captain Vel was recently injured in the destruction of the USS Iowa; she suffered a concussion and third degree burns that required skin grafts. The grafts have healed cleanly with no sign of rejection. She complains of frequent headaches, and scans did reveal a mild case of post-concussive syndrome. Administered analgesic and drovelexin to assist neurotransmitter system recovery and advised subject to return for further analysis if symptoms persist.
Symbiont vitals are on par with Captain Vel - intact and functioning within normal limits, but showing minor physiological signs of stress. She asked after its health; I assured her of its physical health and advised conferring with a Trill specialist regarding further evaluation.

SUMMARY
It is by my medical opinion that Commander Wilson appears to be in ideal health. His blood pressure,cholesterol and breathing rates are all normal/average suggesting he is in good health. The only concern is his BMI ratio which is fairly high however results for BMI tests are not always accurate due to it not taking into account muscle weighing more than fat. Therefore in conclusion it can be considered that he is in very good health (though as all doctors do I'd suggest he would cut off the smoking).

Since her last exam, Lieutenant Commander Zaldo sustained a hairline fracture in the left tarsal equivalency while on leave on the planet Risa. She was properly treated at the time and has suffered no lasting injury. Per Starfleet orders, LCDR Zaldo is still on the correct dosage of the pheromone inhibitor to counter the Eledri Effect.
