Starfleet prioritizes the mental health of their personnel just as much as their physical health and it’s for this reason that the Counseling Department exists. Though their primary purpose is to support officers’ emotional well-being, they are also regularly asked by Command to produce reports on whether an officer is ready to return to duty.
Please feel free to use the report template below as a formal reporting on any counseling RP that your counselor character has recently performed. Though subject was, at the start, afflicted by a very mild and understandable degree of anxiety, subject outwardly presented a distinctively calm and professional mien for the entire duration of interview and was generally fairly well composed. Authority notes that the subject demonstrated a healthy respect and appreciation for the challenges of command, while maintaining a likewise determination to live up to the expectations entailed within. Authority posits that this attitude is likely borne of the subject’s more underachieving origins; whatever the reasons for change, subject seems resolute in his desire to serve and not waste the opportunity he’s created for himself.
While subject does profess to be primarily traditional in command style, preferring a more offhand approach, subject is willing to consider and employ more unorthodox methods of accomplishing tasks, as long as it falls within satisfactory parameters of safety, etc. His externalized world view on others and their motivations trends neutral to slightly negative, though when concerned with only himself, he remains more balanced between optimism and realism. Authority observes that subject largely led interview at the beginning, willing to engage almost immediately on the topic of a recent event in which subject currently questions his judgement. Authority notes that while the subject gave all the appropriate and health-conscious responses to her inquiries, it did have a touch of rehearsedness to it. Authority suspects the subject certainly understands counseling rhetoric, but is still in the midst of gaining distance from the event. As subject seems fairly confident in his ability to gain distance from his dilemmas to relax, authority will not register concern at this time.
Subject appears to possess a fairly moderate temperament, though authority senses that this is due more to age and experience, rather than natural inclination. Periodically, subject demonstrated a willingness to laugh at himself and other amusements, as well as a clear reluctance to elaborate on more painful elements of personal history. Subject’s general outlook is also strongly moderate, trending towards neutral views of just about everything, though slanting a bit positive.

Subject’s outward manner was friendly and sociable from the outset. However, he demonstrated significant trepidation during interview, largely due to his anxieties over how his early career interruptions would be perceived. Subject possesses a fairly deep-seated sense of shame and self-contempt from this which, merited or not, is likely to impact his career’s upward trajectory in the very long term. Though occasionally reluctant, subject responded honestly to questions asked and took complete ownership of his shortcomings. Authority observed that the subject is, despite everything, fairly grounded in his approach to new situations and problems, suffering no lack of self-confidence for his decision-making and demonstrating no particular fear that he will re-lapse.
Subject trends generally positive in outlook and on his perceptions of the motivations of others, with a slightly more external than internal locus of control. Authority is somewhat concerned about the lack of a ready emotional support network; though subject seems to believe he can rely on current family and friends, authority suspects they are likely considered ‘last resort’ supports for him. At start of interview, subject willingly lead with an admittance that she has been feeling unlike herself since her transfer to present posting. Subject has an academic understanding of the benefits of regular social engagement with peers and colleagues, but due to her naturally introverted sensibilities, subject is having difficulty making genuine connections that reflect her interests and comfort level. Subject demonstrates a remarkable level of self-awareness with these observations and her willingness to proactively engage to change an unsatisfactory status quo should be commended. Authority has offered a few actionable suggestions to assist with the subject’s self-professed problems, to which the subject appeared fairly receptive.
Despite the subject’s evident frustration, her mannerisms throughout were generally temperate and collected. Her open displays of amusement, though uncommon, were readily made and established her attentiveness to the discourse at hand. Subject maintains, perhaps unreasonably, high expectations on herself, often feeling as though she is capable of solving any problem with sufficient application of self and unconsciously practicing a double standard of patience between herself and others. Regardless, subject has completely functional working and social relationships with her vessel crew, so authority is confident that if subject continues to apply herself to social situations, she will gain the same equilibrium with her remaining fleet peers as she is.

At this time, authority’s assessment of subject finds her to be a highly polarized individual, given to emotional ends that others don’t tend to reach and capable of very rapid changes in her mood and interactions with others. Subject’s mannerisms are patently transparent and obvious, she either lacks subtlety or completely disdains it, perhaps preferring the forthright approach, and her motivations tend to be fairly evident to others. Her demeanor is particularly warm and friendly which can endear her to some, but may be potentially offputting to individuals who find her to be overly familiar and presumptive.
Subject possesses an extraordinary degree of self-confidence, which authority finds a bit at odds with the feeling that the subject seems perpetually dissatisfied with her lot in life, though not outwardly. Subject is fairly particular about her preferences and especially values demonstrations of trust, both in her as the recepient or her as the giver. Despite the impression that authority has of the subject being under a lot of pressure from multiple sources, subject maintains a remarkable level of moderate to very strong optimism, especially when it comes to her perspective on others and their motivations.

Authority finds it important to first note the potential cultural barriers involved with this evaluation. It is not frequently members of the Romulan Officer Exchange Program are sent to Starfleet Medical for Psychological evaluations. Specifically, some concepts during the final round of questioning seemed rightfully foreign to Subject. Authority has taken this into account and will accommodate to the best of its ability for the purpose of this evaluation.
Subject projected confidence and ease throughout the evaluation, despite an unfamiliar environment and unfamiliar situation. Responses to questions were universally direct and to the point, often prompting follow up questions in order to acquire adequate responses. However, the subject spoke candidly and openly once the right questions were asked, so Authority is inclined to believe the brief answers were evasive in nature.
On the subject of professional relationships and conflict management, the Subject appears perfectly willing to voice opinions, backed by data, when she believed it important but also willing to follow orders when confronted by unyielding command structures, even when disagreeing with said structures. This was reinforced during the final round of questioning where the Subject exhibited a somewhat internal locus of control; Subject believes she is able to directly influence her own environment and her choices but recognizes there are forces over which she has no control over.
When it came to discussing the Subject’s history working with Starfleet, Subject was confident in her abilities to plan and execute plans, but did not seem willing to invest more hope in situations than provided by empirical data, suggesting a more realistic outlook than an optimistic or pessimistic one. This coupled with the Subject’s high discipline would likely predict, at the least, a highly efficient and meticulous Officer.
Subject appeared comfortable with forming social and parasocial relationships with fellow Officers, even those within the Federation and felt confident in speaking easily with those she has established trust with. This results in a social support system usually not seen to develop often between members of the Federation and members of the Republic.

Subject established himself as a highly grounded individual, communicating in a concise manner and generally in possession of a courteous and straightforward bearing. The authority summarizes the subject to be subject to be frank and honest, as well as mentally organized and focused, despite the fact that the subject is also capable of becoming overly engrossed in private thought on occasion as well.
The authority compliments the subject on being particularly objective, and not despondent, in regards to what he perceives as his own shortcomings. While appearing to be primarily mild-mannered, the subject did not hesitate to demonstrate his amusement at relevant points in the conversation. The authority believes that this quality, combined with the subject’s somewhat optimistic attitude towards humanoid cultural development and sentients as a whole, allows for the subject to sustain his calm in the face of surmounting stress.

Subject demonstrated no symptoms of trauma, possibly due to role as physician, which gave her some sense of purpose, even if it wasn’t easy to fulfill. Subject felt somewhat sheltered as the lowest-ranking officer, as the captors’ focus was more on her superiors.

However, authority strongly recommends a period of 2-3 weeks of leave prior to resuming service.
Minimal symptoms of trauma observed, due to subject’s significant time spent incapacitated during imprisonment, which ultimately minimized his exposure to traumatic events. Chief concern is how subject will manage and process the emotions he experienced in reaction to fellow prisoner actions, rather than anything inflicted by the captors.

During interview, subject was generally in possession of a particularly amiable and friendly disposition. Authority has observed that the subject is fairly driven and frequently carries his enthusiasm from task start to finish, a quality that the authority admits seems to be increasingly rare in officers. This passionate streak would explain the subject’s willingness to engage in more dramatic and less rational actions in order to ‘make a point’.
Subject trends strongly towards optimism and positive outlook on both life in general and of the people around him. Subject acknowledges that his inclinations in this matter have resulted in regular disappointment when things don’t turn his way, though that has yet to prevent him from engaging in this behavior. Subject is understandably heavily reliant on the advice of others, though he is likewise capable of studious observation and private conjecture.

Basic Profile Assessment.
Authority finds subject to be of sound mental health and fit for active duty.
During interview, authority observed subject to be a highly agreeable officer, his manner primarily accommodating and easygoing, with no evidence of nervousness or anxiety at the upcoming assessment and readily smiling when moved to do so. In terms of his default reactive attitude to external stimuli, he is chiefly proactive and practically hands-on, preferring to get into the thick of things and learn directly, rather than wait for secondhand information. At the same time, subject finds it unacceptable to not have a complete picture of a situation, so is likewise prepared to consult with others before making a difficult decision.
Authority did observe that the subject may have some difficulty in admitting out loud to traits that he perceives as potential shortcomings, even though said traits are quite commonly shared with other officers and are not necessarily shame-worthy, such as struggling to relax. Subject is likely to be internally more pessimistic than what would be assumed considering his comfortable outward manner, especially when it comes to his own capabilities, while his general outlook towards life and the motivations of others trends strongly neutral to barely moderately positive.

Counseling Routine Exam.
Authority finds subject to be of sound mental health and fit for active duty.
Authority would like to register that a friendly relationship exists with the subject prior to interview and will be utilized to supplement authority’s evaluation of the subject. The subject was recently promoted to Chief Science Officer of Deep Space 13, therefore a complete analysis of subject’s leadership style and conflict resolution (which will be discussed below) was not viable. Authority would suggest a follow-up Command Leadership evaluation in six months to reassess subject’s leadership abilities. Subject admitted to a more ‘lax’ style of leadership, influenced by prior leaders the subject deemed too ‘strict’ or ‘formal’ who may have impacted work efficiency negatively. Subject also noted she prefers a motivational style of leadership, delegating resources to those under her command based on work quality. In regards to allocation of resources from Command, subject demonstrated an understanding of the laypersons she reported to, employing strategies to interest and impress, but not frighten or discourage.
In regards to the basic profile assessment, conducted Stardate 92347.3, Authority finds the subject has undergone emotional and social growth. Subject did not have a patronizing attitude, nor did she stray off topic frequently. At times, subject did elaborate more on a topic than was required, however at other times more elaboration was required. Subject does still suffer from forms of social anxiety, though she is currently undergoing weekly counseling sessions via holochat with an off-station counselor. Subject admitted to having problems picking up on social cues, but she did mention her larger social success beginning with her assignment to Deep Space 13. While the subject may have difficulties making friends, she assigned a high value to friends she does have.

INTERVIEW STARDATE: ((7 May 2016))
Basic Profile Assessment.
Authority finds subject to be of sound mental health and fit for active duty.
Authority would like to register at this time that she has had some trivial interactions with the subject prior to interview, therefore the authority’s impressions and evaluation will be supplemented by these other firsthand interactions. Foremost in the authority’s observances is the marked lack of emotional cues on the subject’s behalf. Though the subject was not completely devoid of feeling, and even self-professed to nervousness or general anxiety, these emotions rarely made any appearance outwardly, which authority feels may create some cognitive dissonance for those that interact with the subject. Subject therefore comes across as blunt and unsubtle, even on occasion a bit patronizing, when authority feels this is unlikely to be the intent.
Authority also feels that the subject may suffer some degree of lack of self-awareness. Though subject has self-identified as someone who keeps her problems to herself, authority found subject to be rather effusive, frequently dipping into personal topics and willing to discuss herself and her prior troubles even when unprompted, albeit in a fairly nonchalant and non-dramatic manner. Subject has a strong valuation for expanding one’s body of knowledge, demonstrating a streak of pride in her position as a forerunner in her areas of expertise and particular readiness in expounding on her research and her opinions with other parties.

INTERVIEW STARDATE: 95142.1 ((Feburary 2019))
Psychological Fitness Evaluation.
Authority finds subject to be fit for active duty contingent upon attendance of counseling sessions.
This evaluation is in response to the communication sent Stardate 95136.5. Authority would also like to note subject has not attended any formal counseling sessions related to the event leading to the requirement of psychological evaluation. Because of this and the subject’s responses to questions throughout the evaluation, authority believes both a return to duty and counseling to be beneficial to the subject. Subject is required to attend weekly counseling sessions for a period of no less than four standard weeks.
Subject displayed signs of discomfort and anger while questioned about the specific event leading to the required psychological evaluation. This was further compounded by frustrations induced by a sense of restlessness experienced by the subject while awaiting a return to duty. While the subject has compensated through a variety of personal projects, the restlessness has still persisted. While feelings of anger and frustration were expressed, subject does not appear to harbor any unnecessary burden of guilt or responsibility for the incident.
Subject has a fairly optimistic view of life and the beings within it, despite professing to have difficulties establishing long lasting friendships. A support system does not seem to exist for the subject, through the subject’s own design. This, however, does not appear to create antipathy or conflict with those the subject works with.
Authority believes once the subject has dealt with the memories and residual feelings connected to the incident, subject will integrate easily into current and future assignments.

Authority finds subject to be of sound mental health and fit for active duty.
Subject presents a particular attitude of someone who successfully powers through long and hard working hours with an enduring focus and steadied pace. She is industriously prepared, perhaps overly so, for potential surprises and her nonchalant demeanor and approach speaks of a readiness to stay on her toes and react calmly. Subject is professionally forthright, open enough with her responses but not offering too much, while also maintaining a low key sense of humor in her interactions. The authority also observes that the subject is thoughtful and cautiously fixated on detail; she appears reluctant or disinterested in assuming too much about a situation without knowing more.
Subject does not have any ambitions other than to stay in her career track, which she enjoys for its clear engagement with causality. Given her somewhat lackadaisical attitude towards experiencing new environments, it suggests the subject is generally open-minded and interested in new experiences if life brings her to them, but is not specifically motivated to pursue them for their own sake. Subject trends moderately positive in both worldly outlook and on the motivations of others and possesses a balanced locus of control.

Basic Profile Assessment.
Authority finds subject to be of sound mental health and fit for active duty.
Subject seems to still be finding his feet to some degree with his relatively recent promotion. While subject does not evidence any significant self-esteem issues, neither does subject carry himself with any particular confidence. During interview, subject was somewhat nonexpressive and though his answers were not exactly terse, they were fairly plainspoken, straightforward, and light on details. Subject values the welfare of his crew, as well as his loyalty to Starfleet and though he professes to be occasionally susceptible to interpersonal conflicts, he does his best to navigate them with equanimity.
Subject seems to have a substantial grasp of his emotional health and support network. He trends towards suspicion and pessimism when taking into consideration the motivations of other people, which runs parallel to his feeling that individuals have very little control of their external circumstances. However, the subject does ascribe a great deal of faith and optimism in the effectiveness of his own controls and actions, which suggests an incongruity in self-perception.

Subject presented an impeccably disciplined and competent manner during interview, without ever stepping so far as to be seen as brusque or unceremonious. Subject’s largely pragmatic attitude was in evidence in just about all of her responses, with a clear preference for methodologies and actions that are transparent and cooperative in nature and strongly disliking unnecessary complications of any kind. Though she self-describes as a traditionalist in her approach to problem-solving and management, subject is not unappreciative of experiences that will expand her world view and values such memories as unique imprints upon her psyche, leading the subject to a seemingly well-balanced sense of self.
The subject’s world view at large remains moderate, neither feeling that circumstances are too beyond her control or unrealistically well within them. Subject’s outlook on other individuals and their potential motives is, perhaps, guardedly positive. Though she continues to be resolved in keeping her personal life as strongly segregated as possible from her professional one, subject maintains a healthy self-assurance in regards to her own capabilities and a similar degree of confidence in the abilities of her support system.

Subject professes to being remarkably satisfied with his posting with the fleet, feeling both pleased with the relevant leadership attitudes and contented with his particular standing and the work that has brought him there. Authority finds subject to be of sound mental health and fit for active duty.
During interview, the authority observed the subject’s manner to be generally congenial and open, moved easily to smiles and willing to discuss her history. Though subject initially appeared to be displeased with the necessity of being evaluated, the authority is grateful to note that the subject did not appear to be recalcitrant in any degree, though this did not preclude the subject from occasionally expressing amusement whenever the authority required clarification on any topic. Authority additionally observed that the subject appears to have a remarkably conscious connection with her symbiont, which may seem somewhat chaotic to the outside observer, but subject seems wholly comfortable with the arrangement. Authority notes that this internal preoccupation has been reflected into external behavior, as the subject rarely remained physically idle during interview.
Of particular note is the subject’s extraordinarily practical outlook towards the majority of subjects, which is at least partially due to the advantage of multiple lifetimes’ worth of experience. The subject is capable of switching rapidly from one mood to another as need dictates, with the finely honed instinct for when to mourn and when to move on.
