The reports for Captain Fives are in. Unfortunately, the prognosis is not well. The patient is suffering from a full implant rejection; the implants on her right arm have failed completely and other implants are close to failure. Her nanite levels are now at 50% and still dropping; the IV and electrical drips are ineffective at stopping the nanite loss.

Upon consultation, the courses of action are listed below, in order of recommendation. Any solution would have to be confirmed by the patient beforehand.


  1. An extensive operation to remove some/most/all the implants in the patient's body. We would aim to remove as little implants as possible and try to administer medication to aid the body into adapting to the implants. Following the operation, medication will have to be administered to the patient so that her body can adapt to function without the implants.

    However, there is a substantial risk here in that her body may not be able to function without the implants, causing her to go into shock and eventual death. Once the implants are removed, it would be very difficult to go back.

  2. A creation of Five specific implants, attuned to the body of the patient. This would require her implants to be removed and replaced by the new implants, which would have to be specially made. The problem with this is time; the longer it takes for the patient to receive treatment, the less likely we are able to save her. In order to create such personalised implants, it would take time the patient does not have. Should these implants then fail or her body rejects them, the patient will be in a worse condition than she is in already.

  3. Reconnect her to the collective. The Borg have the ability to fix problems in their drones. We might be able to recreate a collective within a controlled environment in the starbase, using other Liberated drones, but this is a long shot. The other alternative is to let her get reassimilated into the collective and then hope we can rescue her afterwards.


I don't think I need to tell you about the dangers of option three, but I have put it in there so as to give you a full idea of what options we have. I've CC'd in Command because to pursue options two or three would require executive approval.

Dr. Judur,

Per your request, I sent a consultation request to Starfleet Medical's specialists in Borg liberation. Their conclusions regarding her implants were the same as your own; remove, replace or repair.

Precedent suggests attempting strict removal wouldn't give us the results we need. Her body has been dependent on these implants for decades; it would take intense medical and physical therapy to help her cope with losing a single piece of equipment, let alone the whole set. Ladling systemic shock on top of that mess could be lethal.

Similarly, we have no way of knowing what reconnecting her to the Collective would do to her mental state, and we have no guarantees that it would solve the low nanite production or the implant rejection.

I recommend replacement of the implants over a period of time with tissue-matched synthetics, coupled with aggressive immunosuppression therapy to keep the rejection symptoms in check. If you agree, the liberation specialists are willing to work with you to design the new implants.

It's an ugly situation, Artrin, but I think it's possible. Let me know what you need.

Apologies for the delay in response; I did not want to message you without doing a bit more research in this matter beforehand. Working with the liberation specialists, I believe we have come up with tissue-matched synthetics that could work with the minimal amount of immunosuppressants needed. We are unsure how long the synthetics will last, but it is a step forward if anything and buys us time to come up with another solution if needed.

We are ready to begin the surgery as soon as possible. The surgical procedure is has been projected to last around twelve hours (from the three simulations we ran over the last week), barring any complications. I will be the lead surgeon for this operation, accompanied by Doctor T'Rella, an expert in Borg Liberation surgery. You are welcome to be present during the operation, but I'm afraid I will have to suggest you remain in the viewing area, so as not to crowd the operation area. I also have a standby team to take over the operation so the staff do not get fatigued, and a reserve team too in preparation for if there are some unforeseen problems.

Operation Room Three is being prepared for the procedure. I have taken it off the reservation lists so that there is no chance of a double booking. I would also like to request a security team and Borg resistant restraints during the operation; I want nothing left to chance.

We should be prepared in the next 36 hours. Attached are the Operation Forms, signed by myself and Doctor T'Rella. As is standard, we require your (or any Command Staff's) signature and that of the Captain before we can proceed.

I came across this case file within the 36 hour preparation time and checked the surgery schedule; it seems that the operation has not yet been performed. I have extensive knowledge of and interest in cybernetics, with a doctoral degree from Shirkar Academy on Vulcan. Optimally I would like to participate in such an operation, but I understand that I am only a junior physician in Starfleet and has not worked with this patient previously, so there will be others more suited to this task. However, I would still like the opportunity to view this surgery as I believe it will further my knowledge on my field of specialty.

More so, I have read up on the past records of this case and understand that this operation may only be a temporary solution. I hope that the patient will be well without the need for further medical intervention, but should complications arise, I would like to offer my expertise on artificial bodily replacements.

The operation on Captain Jager was successfully completed earlier today. We went over our anticipated time by four hours due to complications in the surgery; her body was showing signs of failure due to the removal of certain implants and we were in critical condition for a long time. However, all the staff performed excellently and we were able to replace all of the affected implants, totalling to around 83% of the entire implants in the subject's body. Captain Jager is currently resting and we will begin to see what effect this has on her in the next few days. So far, her body is showing signs of rejection which we are combating with immunosuppressants but this was to be expected.

I just want to personally thank Dr T'Rella, who was with me during all 16 hours of the operation. Her experience and skill saved the Captain's life many times.

I will keep you updated.
