LTJG Niraj requested a Medical Check-Up for their Facial Implant. Stated ‘Mild Discomfort’ and other subjects of irritability.
CONCLUSION
Once all the issues were sorted and more info was given on the implant itself, the decision was made to leave the implant be for now until the Patient decides if she would like some form of surgery rendered in the future; and was prescribed a month-long low level painkiller until then (2cc Morphenolog; once daily if pain occurs.)
SUBJECTIVE FINDINGS
Patient states facial implant has been causing mild discomfort and other minor issues such as headaches and occasional itching that goes away after an uncertain amount of time. Says the issues have only started within the past week or slightly prior with no supposed cause such a blunt force or similar; notes no rolling over in sleep either that could cause an issue. Otherwise, patient is in excellent health and would be considered perfectly healthy if this issue were not present.
OBJECTIVE FINDINGS
Use of a tricorder shows that the implant itself was pressing against the Ocular Nerve; perhaps from a bracket in the implant being tightened too far during installation and slowly digging itself in over time as the skin wears down. Throughout the course of the talk with the patient, she stayed exceptionally calm and agreeable; no signs of duress or aggravation throughout.
PLAN OF CARE
Month-long prescription of Morphenolog (2cc) in the form of a daily dose when pain occurs.
During a Medical Check-Up for patient it was found that she didn’t have a Medical Evaluation on file, using this time, we went ahead and rectified this issue as well.
CONCLUSION
Patient is in excellent health and meets all standards of physical fitness. She has passed all testing and evaluation necessary for medical personnel and outside of the current mild implant discomfort discovered just after has no significant medical conditions that bear review.
SUBJECTIVE FINDINGS
Patient has stated a possible allergic reaction to a ‘Aldebarran Mud Flea Virus’ when younger; unknown if this has persisted. Otherwise states feeling in good health and no injuries to note.
OBJECTIVE FINDINGS
Patient’s vitals and statuses all read as normal. No issues outside of implant discomfort which is to be disregarded for the sake of this Eval
PLAN OF CARE
LTJG Nirajis rated capable of active duty without reservation at this time. Implant discomfort is once again being disregarded as of now and will be handled on the patient’s own time; please reference the Above Report for further information
Baseline Medical Assessment
CMDR Barron placed a request of a Standard Medical Evaluation for service within 38th ‘Argo’
CONCLUSION
CMDR Barron is in excellent health and meets all standards of physical fitness. He has passed all testing and evaluation necessary for medical personnel and has no medical conditions that bear review.
SUBJECTIVE FINDINGS
Patient does speak of having mild insomia; along with muscle cramps in the lower right calf and right mandible. Says it can be handled by light hyper-extension of muscles for ‘a minute or two’ and otherwise states fine.
OBJECTIVE FINDINGS
Patient’s condition during was exceptionally stable; vitals were of no issue, tricorder and bioscans indicated light strain in the areas patient was aware of, but nothing of concern. Injuries themselves may be handled with light painkiller, but that is left up to the patient.
This is a routine follow-up for Captain Tungsten to check on his healing progress from an injury he sustained prior to Shore leave.
CONCLUSION
Captain Tungsten is fully cleared to return to normal duties
SUBJECTIVE FINDINGS
The Captain expressed slight stiffness in muscles and movement but nothing that prevented day to day activities
OBJECTIVE FINDINGS
Bio bed results were negative for anything that may be the cause of the stiff muscles and was negative on any lingering issues within the afflicted shoulder.
PLAN OF CARE
It is recommended Captain Tungsten perform light stretches for the next week to work out the muscles, and though he is cleared for regular duty again, some daily time on the holodeck or gym may be useful to re-strengthen the limb.
PATIENT’S NAME: SGT Ten of Sixteen
APPOINTMENT STARDATE: 97603.8
PURPOSE AND CONTEXT OF EVALUATION
Medical Routine Exam
Patient’s CO, LCDR Henry Hook, is missing. A recovery is being planned, but it is anticipated that LCDR Hook has been contaminated with a fungal agent as yet untreatable by conventional means.
Given the short window of opportunity to treat this infection before it becomes critical and the lack of information on the pathogen itself, SGT Ten of Sixteen asked to explore the possibility of utilizing their nanoprobes as a potential cure and stabilizer for LCDR Hook’s condition.
CONCLUSION
SGT Ten of Sixteen remains fit for duty without reservation.
SUBJECTIVE FINDINGS
The patient came in for an exam to discuss the possibility of reactivating their nanoprobes to use them as a treatment for a fellow officer.
OBJECTIVE FINDINGS
SGT Ten of Sixteen carries the same nanoprobe load as other liberated Borg. Utilizing liberated Borg nanoprobes in this fashion is not unprecedented; several documented cases prove that, in the short term and with the appropriate bioengineering supervision, these nanoprobes can be used as a powerful panacea for many races and many medical conditions. This officer determined that reactivating Ten of Sixteen’s nanoprobes while they remained resident was too dangerous to consider; the interactions of the active nanoprobes with the existing Borg technology still installed is too great an unknown.
As an alternative, with Ten of Sixteen’s permission, a blood sample including inactive nanoprobes was drawn. The sample was exposed to the activation sequence and the nanoprobes within it were verified as reactivated. This sample and a separate deactivation serum were given to Ten of Sixteen in anticipation of use for LCDR Hook.
PLAN OF CARE
If LCDR Hook is recovered and infected with the Drauth III fungus, the injection of the active nanoprobes should offer a possible cure for the infection; however, once Hook is stabilized and free of the fungus (or the nanoprobes prove ineffective), the deactivation serum must be administered as quickly as possible to prevent expansion of the nanoprobes within the new host.
Baseline Medical Assessment
LTJG Naderi placed a request of a Standard Medical Evaluation for service within 38th ‘Argo’
CONCLUSION
Patient is in excellent health and meets all standards of physical fitness. She has passed all testing and evaluation necessary for medical personnel and outside of the current mild implant discomfort discovered just after has no significant medical conditions that bear review.
SUBJECTIVE FINDINGS
Patient made no comments of personal injury issues; believes to be in good health and having no injuries.
Though Ensign Alkitor had an elevated heart rate, it is this officer’s belief that the Ensign is fit for duty.
SUBJECTIVE FINDINGS
Patient believes themselves to be overall in good health
OBJECTIVE FINDINGS
Patient is in fact, overall in good health. This officer found no illness or injury that would preclude the patient from performing their duties. However it should be noted the patient did have Tachycardia, or an elevated heart rate. When questioned about it, the patient gave assurances it was from a distates in being ‘poked and prodded’
PLAN OF CARE
Though Tachycardia is not inherently dangerous among humans, prolonged episodes of it can lead to serious issues. It is this officers expert opinion that the Ensign should monitor himself for continued abnormalities in his heart rate. Should the issue continue or worsen, other avenues may have to be explored.
PATIENT’S NAME:
CMDR Akaela, Tala
APPOINTMENT STARDATE:
97647.1
PURPOSE AND CONTEXT OF EVALUATION
Medical Routine Exam
CMDR Akaela requested a Annual Medical Assessment.
CONCLUSION
Patient is in excellent health and meets all standards of physical fitness. They have passed all testing and evaluation necessary for medical personnel and have no significant medical conditions that bear review.
SUBJECTIVE FINDINGS
Patient believed to be in good health, stating that their visit was ‘For the records’ after having been gone for so long. Beyond that, patient did request a very low-dose hypo for minor eye strain and/or headaches, as they are currently stationed aboard a Romulan Vessel.
OBJECTIVE FINDINGS
Patient was in stable condition during and displayed no issues during scans; however, equipment did have to be temporarily adjusted to ensure that no false readouts would occur- something that had to be explained by the patient themselves, which can be rectified by adding it to their station Personnel File.
Patient has a form of microorganism that exists on their person that is part of a symbiotic bond with their body. While the patient had idle worries of the health of such organisms, scans did help relieve such worries; as all microorganisms were in as good of health as they were.
PLAN OF CARE
Patient was given a considered OTC Low-Dose Painkiller (2cc Morphenolog) that will last for a month or thereabout.
PATIENT’S NAME:
Captain Hen Dao Shin, MACO ( @Kelimnac )
APPOINTMENT STARDATE:
97652.5
PURPOSE AND CONTEXT OF EVALUATION
Baseline Medical Assessment
CONCLUSION
Patient was evaluated with multiple levels of medical scrutiny. No treatments were necessary, and all tests and assessments were passed. Additionally, no medical conditions were discovered during the evaluation. Patient is hereby medically cleared for duty.
SUBJECTIVE FINDINGS
The patient stated he was in good health, but expressed a willingness to ensure there was nothing he couldn’t openly feel. His only complaint was for eye strain, but he did not request a treatment for it, stating that rest should be sufficient. Upon being informed of his significant muscle strain following his recent training experience, he readily agreed to perform light duties for two weeks until his muscles have had time to rest. Overall, patient is medically cognizant of his limitations.
Notes on the patient’s own perception of their condition, including complaints, symptoms, and impressions.
OBJECTIVE FINDINGS
Patient reported in stable and alert condition, and remained so throughout the examination. All vital signs were normal, and scans indicated nothing significant to report. Scans did indicate two minor issues that were discussed - evidence of multiple bone fractures and strained muscles. The fractures did not leave any permanent damage, however, and appeared to be fully healed. The strained muscles were addressed separately, though no prescription was necessary.
PLAN OF CARE
Two weeks light duties to allow for proper physical rest and recovery following the patient’s recent training environment. Follow-up if issues remain after the light duty period.
PATIENT’S NAME: Sedai, Katriel E.
APPOINTMENT STARDATE: 97764.5
PURPOSE AND CONTEXT OF EVALUATION
Medical Fitness Evaluation - Ordered by Admiral Bishop following patient’s return from 15 days marooned on an unexplored planet.
CONCLUSION
Deemed fit for duty without reservation.
SUBJECTIVE FINDINGS
Patient reports no complaints. Patient denies chest pain, shortness of breath, headache, dizziness, and GI disturbances.
OBJECTIVE FINDINGS
Patient alert and oriented, speaking in full sentences, breathing without difficulty. No trauma noted. Cardiopulmonary function intact. Neurological function intact. Psionic function intact. Vital signs WNL attached. Lab values WNL attached.
PLAN OF CARE
Patient received prior treatment for minor injuries upon her return. No residual deficits noted. Antibiotic prophylaxis on file. No follow-up care required.
Medical Physical Evaluation following patient’s return from 15 days missing in action. Physical ordered by Admiral Bishop prior to patient RTD.
CONCLUSION
Standard and detailed medical scans in addition to a full lab workup revealed nothing that would hamper the patient in her duties. All wounds sustained during the patient’s time marooned are healing without issue, and no abnormal issues presented themselves in labwork or scans.
SUBJECTIVE FINDINGS
Patient expressed concern for her shoulder following her injury, and complained of soreness. However, the patient denied any pain and still had a full range of motion. Scans with tricorder and biobed indicated no issues with healing process. No other issues were reported.
OBJECTIVE FINDINGS
Tricorder and biobed scans revealed no cardiac, pulmonary, neurological, GI, skeletal, or muscular issues. Patient was alert and cognizant to her surroundings at all time. Vitals and labs samples were taken - all values normal.
PLAN OF CARE
Two weeks light duties after return to duty to avoid straining of prior injury. Recommendation of physical therapy to ensure any previously-damaged muscle can be nursed back to full strength. No additional medication prescribed.
CMDR Na’Toth is rated fit for duty without reservation.
SUBJECTIVE FINDINGS
The patient came in for the annual required physical exam.
OBJECTIVE FINDINGS
CMDR Na’Toth, despite her age and a considerable collection of former injuries, is in excellent physical health.
PLAN OF CARE
Though she is in good physical condition, like most entities who have reached the latter half of middle age, she could benefit from a nutritional supplement and, like many officers in command positions, a steadier supply of sleep. She has also expressed an interest in potentially regenerating her eye, so I provided information on the process involved.
Detailed scans via the use of Tricorder and Biobed revealed nothing of immediate concern that would hinder the patient from carrying out his duties as a Starfleet Officer
SUBJECTIVE FINDINGS
The patient expressed they were in good health and had no pressing concerns medically speaking oh their health
OBJECTIVE FINDINGS
Tricorder and biobed scans revealed no cardiac, pulmonary, neurological, GI, skeletal, or muscular issues. The patient was alert and cognizant of his surroundings at all times. Vitals and labs samples were taken - all values normal.
PLAN OF CARE
Though there is nothing of Immediate concern as noted, This officer advises the Captain to look into a physical therapist to assist in strengthing the muscles and bones around old wounds he had sustained. An hour of light stretches could help loosen up any problematic scar tissue developing beneath the skin.
LENGTH OF APPOINTMENT: # 1/2 hrs

PATIENT’S NAME: LTJG John Fitzgerald Sandford. ( @Loxton )
APPOINTMENT STARDATE: 98020.3
PURPOSE AND CONTEXT OF EVALUATION
Medical Incident
CONCLUSION
The Lieutenant was remarkably drunk despite having been acting as a bartender for an off-duty social get together comprised of junior officers.
SUBJECTIVE FINDINGS
The lieutenant was close to suffering alcohol poisoning by the time we managed to get to sickbay. Slurred speech, motor movements of lower extremities failing punctuated by periods of high emotions and reflecting on his past. Biobed scans showed clear indications of alcohol saturation of the liver, lowered blood pressure, and raised pulse.
OBJECTIVE FINDINGS
The patient was immensely drunk, flushed, emotional, and prone to potentially falling down
PLAN OF CARE
I administered two cc’s of anti-intoxicant to the LT. It was not enough to instantly cure him of intoxication, however, it would be enough to at least alleviate the hangover. I recommend a follow-up and heavy consumption of nonalcoholic beverages. The patient is allowed to return to duty once he feels better.

The patients were alert and aware of their injuries and the circumstances surrounding their injuries
SUBJECTIVE FINDINGS
The patients all sustained lacerations and contusions to their upper torso with Captain Serah sustaining lacerations to their right hand running parallel to the metacarpal
OBJECTIVE FINDINGS
Patients were all relatively lucky their injuries were not more serious considering the circumstances. I am pleased to report that all three of the officers are cleared to return to duty
PLAN OF CARE
I want the officers that were injured to follow up with me in the next few days so I can check in on their progress, otherwise, they are free to return to duty

From my scans, I was able to spot a severe fracture to the Centurion’s left arm. It has been fully repaired by whatever medical professional she has worked with in the past. I also found evidence of residual scar tissue near vital organs suggesting a massive load was placed there at one point. This too seems to have healed without any issue as well.
