Sirs,
I failed to submit a proper request for leave to travel to Turkana IV in order to investigate my parentage. Given the dangers inherent with this endeavour, and the colony's antagonistic stance towards the Federation, it was my desire to complete this expedition as expediently as possible, and without endangering any of my crew or fleetmates. In my haste, I neglected to send the form I'd completed. It is my understanding that my XO, Lieutenant Commander Corspa Sh'thaal, located this form and provided it upon the U.S.S. Windrunner's return to Deep Space 13.
I take full responsibility for my actions, and am prepared to deal with any ramifications you deem fit to apply to said actions.
I will be in recovery from injuries for the next 2 days. Please contact me with any questions or to update me on my status as a member of Gemini Squadron and 38th Fleet 'Argo.'
