Commander,

As discussed, here are the terms of your treatment plan, in writing.

Therapy and Social Education Plan:

You will attend ONE therapy session for one hour once a week, to be scheduled in advance with my office.
You will attend ONE Federation cultural education class per week, these classes are two hours in length and are held on Saturday nights at 17:00.
You cease taking the previously prescribed Fancinol immediately.
You will begin taking the newly prescribed Darnoxynil B starting tomorrow morning, and as long as deemed necessary for anxiety.
You will follow the prescribed tricorder etiquette.

Tricorder Etiquette:

From this point forward, you will carry ONE tricorder with you, outside your quarters, for duty purposes only. This tricorder will only be for your duty tasks and will not be used otherwise, unless ordered by a superior.

You will not bring this tricorder into any of the social areas of the base, unless ordered by a superior.

Your will not scan fellow officers, crewmen, cadets, Republic Navy personnel, civilian contractors or guests, unless ordered by a superior.

You will leave your tricorder in your workspace during breaks, unless ordered by a superior. If you forget, you will leave the device with security personnel before entering any social areas, and reclaim it upon leaving.

You will submit yourself to bodily search, if you are suspected to have broken any of the above mentions tricorder related rules, unless ordered by a superior.

Patient Rights:

You have the right to take up any issues with my Treatment Plan with my department head, or the Base CO.
You have the right to request therapy from another counselor, knowing that this plan will be forwarded to them with my recommendation that you follow it.
You have the right to refuse treatment, knowing that this may impact your duty status and potentially your fitness for continued service in Starfleet.

Counselor,

I accept the treatment plan and will abide by it's regulations and requirements. I request that during the time of treatment I be allowed the ability to briefly return to my home-world to preform religious and social rituals, honoring the death of my offspring and increasing in tribal status. During this proposed four day leave I will take the medications, and to help with the further treatment I will still abide by the regulations put unto me.

I will give my recommendation to your request for leave. Though you will have to send a leave request chit to Captian Perim before you can be granted leave. I would also suggest you consult with the medbay as to your health before such a leave should be granted.

Fleet Captain,

After speaking with Commander Osiangli, I am comfortable with a conditional return to duty for him.

While he still needs continued therapy, medication and cultural training, he would be better served with daily duty, and the Science department will benefit from having him back.

He taking his leave this evening, once he returns, I recommend he be allowed to return to his duties.
