A delta shift work crew removed some sort of biowaste from the wall cavity left by the removal of the non-holographic fireplace. A second work crew then sealed off that section to prevent further access.

Rumour has it the U.S.S. Dallas was seen briefly on Starfleet monitoring stations in two places over 10 lightyears apart, before the first vessel appeared to vanish. Surely this is just the result of a malfunctioning subspace relay.
